John Daly has been suspended five times and cited 21 times for not giving his best effort on the PGA Tour. Those are among details contained in a 456-page disciplinary file that has become public record through a lawsuit. The Florida Times-Union obtained the file, which the PGA Tour had to give Morris Publishing Co.
