This is a Canadian documentary series about aviation incidents. It's been in production for almost 10 years and airs on various channels throughout the world (I think you can catch some episodes in US on Discovery or NatGeo).
What's really impressive about this show is that it is a true documentary, focusing on the circumstances that led to the crash (or incident) and the subsequent investigation as opposed to simply dramatizing the events like so many similar shows do. In fact, many of the incidents depicted don't necessarily result in a crash, but they are significant from a safety standpoint.
If you enjoy investigative documentaries, I highly recommend this show. You will also learn a lot about aviation and flight mechanics.
This show is not always easy to find on TV, but there are some DVD and Blu-Ray releases and some torrents if you prefer to go that route.
