There's some real time in the planning of heists and such, and in the lottery and the scratchers, but the execution is all text based.

Which was a bad idea. She hit the driver over the head and he lost -5 Health. After you crashed you were knocked out cold and the hotchick stole -16789 Money.

********! I haven't had a successful heist in a while and people are getting millions in them.
