I know of one person who actually successfully got himself removed from a list (and stayed off). Keep in mind, this was probably close to 10 years ago, I don't know if you can still pull this off.
Context: He has tried to get himself removed off lists before but it's never stuck. He was getting fed up and decided to escalate. The key (as he tells me) is to remain super calm and don't get angry.
He gets a call from a telemarketer. He was super calm, played along, answered a few questions. Nonchalantly, he asked what company the telemarketer worked for. Feigned interest in starting a career, claimed he was a good people person, yadda yadda yadda.
He got the name of the company, and found the name of the CEO and several other high ranking officials. This part I'm a bit hazy on, I believe he called the company (it was a Canadian company located in Montreal) itself and got the extension of said CEO and basically called her a couple times until they put him through.
He calmly stated that he does not appreciate being called for offers and would like them to stop. The CEO was pretty shocked and angry that he called her at work and just hung up on him. He called again the next day from a different phone and calmly stated that he called the day before about getting removed from ALL lists but did not get a chance to leave his name. Again, the CEO hung up on him in anger before any real conversation took place.
Now he took it to the final step, he somehow tracked down her home phone and address (I'm pretty sure from a phone book but i'm not sure). He calls her up that evening at home and again, calmly states his desire to be removed from the list. At this point the CEO capitulates, takes his details and he hasn't been on a list since (well, at least up until I last talked to him 3 years ago).
It's the stuff of legend within our circle of friends, and we love telling it to anyone who hasn't heard it. Like I said, I'm not sure you can pull this off these days, even back then he said he was super sketched out thinking he had gone too far.
